Big Stone County Local Advisory Council on Children’s and Adult Mental Health is a proactive group whose mission is to improve the delivery and quality of mental health service for adults and children. The Council will promote education, coordination, and advocate to assist individuals in managing their mental health, as well as work to increase awareness of mental health issues and needs.

The Southwestern Minnesota Adult Mental Health Consortium is a Joint Power entity of 18 counties in Southwestern Minnesota.  The mission of the group is to develop, participate and strengthen a continuum of care providing the “right service” at the “right time” in the “right place” to support mental health recovery.  SMAMHC is committed to the partnerships and collaboration built with the Counties and Providers in the Southwestern 18 County region.  Working together to inspire, provide hope, empowerment and overall wellness.  Programs supported by SMAMHC that benefit Big Stone County residents include housing supports, Assertive Community Treatment, and Intensive Residential Treatment Services.

Mental Health Case Management helps adults with a serious and persistent mental illness (SPMI) and children with a severe emotional disturbance (SED) gain access to needed medical, social, educational, vocational, financial and other necessary services as they relate to the recipient’s mental health needs.

To make a referral for case management call Big Stone County Family Services at (320)839-2555. If you are unsure of eligibility, please complete the referral and allow the agency worker to connect with the individual. Big Stone County will assist in the coordination of appropriate diagnostic assessments and eligibility determination.

Individuals who are disabled and have complex needs (including but not limited to mental health) that require long-term services and supports may be referred for a MnCHOICES assessment. This assessment helps to match services to an individual’s strengths, preferences, and needs. A person of any age can be referred by calling Big Stone County Family Services or the Disability Linkage Line at (866)333-2466.

(Healthcare Professionals Only) CSP helps people to improve and maintain function in their homes or community. Services are provided individually or in groups. Drop in centers provide activities to promote socialization and leisure opportunities. The CSP provider for Big Stone County is Woodland Centers. The purpose of ARMHS is to provide mental health rehabilitative services to people with serious mental illness using evidence-based and best practices to foster recovery and self-sufficiency. The focus of the services is to regain and restore lost skills or capabilities as a result of mental illness. Mental Health Practitioners work with recipients to establish personal meaningful goals based on their vision of recovery to overcome the barriers of mental illness. Clients meet with a mental health professional and are taught skills to assist them with issues such as anxiety, depression, suicidal thinking and relationship problems. Screening is provided 24/7 by calling (800)992-1716. This service is provided for adults and youth. Woodland Centers Crisis Unit is in Willmar Minnesota.

Groups focus on Illness Management and Recovery where people develop personalized strategies to manage their mental illness and move forward in their lives. Skill Training based on Dialectical Behavior Therapy is also offered. Skill training group is designed to treat people with severe dysfunction in coping, emotion management and interpersonal relationships. Individuals may be referred by calling Woodland Centers or Big Stone County Family Services. Mileage reimbursement may be available for individuals who are on a Minnesota Health Plan. For more information regarding mileage reimbursement, individuals should contact their financial worker at Big Stone County Family Services.

(Healthcare Professionals Only)IRTS are time-limited mental health services provided in a residential setting. Recipients of IRTS are in need of structure and assistance from 24-hour mental health staff and at risk of significant functional deterioration if they do not receive these services. IRTS are designed to develop and enhance psychiatric stability, personal and emotional adjustment, self-sufficiency, and skills to live in a more independent setting. Eligible recipients must be 18 years old and meet the IRTS admission criteria. Recipients may receive IRTS instead of hospitalization, if appropriate. (Healthcare Professionals Only)ACT is an intensive, comprehensive, non-residential rehabilitative mental health service team model. Services are provided by multidisciplinary, qualified staff who have the capacity to provide most mental health services necessary to meet the recipient’s needs, using a total team approach. The services is provided to recipients with a serious mental illness who require intensive services and are offered on a time-unlimited basis 24 hours a day, 7 days a week, 365 days a year. To be eligible the recipient must be:

• • 16 years of age
• • Eligible for MA
• • Have a primary diagnosis of a serious mental illness as determined by a Diagnostic Assessment
• • Be a member of the target population the ACT team serves
• • Have a LOCUS assessment with a Level 4 indication
• • Have a completed Functional Assessment following the domains specified by statute with three or more areas of significant impairment in functioning.

Services provided by ACT include: case management, support and skills training, illness education and medication management, assistance in locating and maintaining safe and affordable housing, and psycho-education for family members. County case management is not provided while a recipient is receiving ACT. The Big Stone County ACT provider is Woodland Centers, for more information call (800)992-1716.

(Healthcare Professionals Only)Beginning July 1, 2016, behavioral health home services are a Medical Assistance covered service in Minnesota. The health home model is a provision of the Affordable Care Act and is available to states to serve the needs of complex populations covered by Medicaid. Health home services are required to provide six core services: Comprehensive care management, care coordination, health, and wellness promotion, comprehensive transitional care, individual and family support, referral to community and social services. To be eligible a person must have a serious mental illness or emotional disturbance as defined in MN statute. Duplicate services are prohibited; therefore, a person is not able to receive behavioral health home services and mental health- targeted case management, ACT, vulnerable adult-targeted case management or health care homes care coordination. In Big Stone County, Woodland Centers should be contacted for more information about these services. It is suggested that contact with Big Stone County Family Services be made as well to ensure that services are not already being provided before making a referral for a Behavioral Health Home.
