OAHS New Hire Benefits Presentation

Below you will find the benefit enrollment forms.

As you complete your forms, please print and sign/date them. Once you are completed, please turn in your packet to Human Resources no later than November 16, 2018.

Required Forms:

Health Insurance
Dental Insurance
Life Insurance and Short-term Disability
Long-term Disability
Flexible Spending Account (FLEX)
Permission to use image


Optional Forms:

OAHS Christmas Savings Account – If you wish to participate, complete the form and return to HR.

PTO Cash-out Request Form – If you wish to participate, complete the form and return to HR.

Annual Physical Verification Form – This form will need to be completed and returned to Human Resources if you participate in the wellness program for health insurance. If you do not participate in the wellness program, you do not need to return this form.
